DO YOU TAKE INSURANCE?
NO. You may have noticed by now in your search for a therapist that a good many do NOT accept insurance plans, and that is also the case with me. Why??
A number of reasons actually. For one, it can cause confidentiality concerns. It is my personal belief that therapy should be held as sacred and private as possible, yet in order to process a client’s claim, I must provide certain personal information about him or her which may affect their employment etc. depending on where they work.
Furthermore, insurance companies typically require a mental health diagnosis and will often determine the type of treatment and duration of services allowed for a specific diagnosis. I refuse to be limited and so should you.
Your health and wellness is your business and no one else’s.
Upon your request however, I can provide you with a special statement —referred to as a superbill or CMS-1500 form— for you to submit for reimbursement to your insurance provider. I make no guarantees about the level of reimbursement you will receive from your insurance company.
I suggest you contact your insurance company, either online or via the member services number located on the back of your insurance card, to ask questions about out of network coverage as they pertain to your plan so that you can make an informed decision before beginning therapy.
Some HSA/FSA accounts also allow counselors to bill directly. Please contact your insurance provider for more info on that as well.
WHAT QUESTIONS SHOULD I ASK MY INSURANCE COMPANY ABOUT OUT-OF-NETWORK BENEFITS?
- Do I have out-of-network mental health benefits?
- Are services rendered by a licensed mental health counselor covered? My license in the state of FL is MH14199.
- Is any pre-approval required before obtaining out-of-network mental health services in order to be reimbursed?
- If I have out-of-network benefits, will I be reimbursed the full amount I paid or a portion?
- Do I have a deductible and if so, what is it?

WHAT ARE THE PAYMENT OPTIONS?
I accept checks, cash, and all major credit cards. as well as PayPal. I can also accept HSA/FSA cards. Checks for services should be made payable to: Arya Therapy Services. Change cannot be made, so any over payments will be credited to your account.
WHEN IS PAYMENT DUE?
The full fee for your session is collected at or before the time of service via cash, credit card, debit card or check. We can also securely record a debit or credit card so you will not have to take time for payment during your session. I do not bill for services, or carry account balances.
WHAT IS YOUR CANCELLATION POLICY?
Your appointment time is established and held especially for you. If you miss your scheduled appointment, and you do not cancel forty-eight hours in advance, you will be responsible for the full cost of that appointment.
Fee for insufficient funds or declined card is $25.00. You will not be allowed to book another appointment if you have an outstanding balance. Outstanding balances not paid within 30 days may be sent to collections.
DO YOU OFFER DISTANCE COUNSELING VIA PHONE OR VIDEO SESSIONS?
Yes, absolutely! Learn more about how this is done here. While insurance may reimburse for psychotherapy sessions conducted in my office, please note that sessions via phone/video conferencing will likely not be eligible for reimbursement.
WHAT ABOUT EMAILS, RECORDS, & OTHER NEEDS?
In addition to regularly scheduled appointments, I retain the right to charge for other professional services you may request.
Other services include report writing, telephone conversations lasting longer than 15 minutes, attendance at meetings with other professionals you have authorized and requested, preparation of records or treatment summaries, and the time spent performing any other service you may request of us.
These fees will depend on the nature of the request.
- If you elect to email, text, or call me about a situation outside of therapy and do not wish to wait until your next appointment to have us explore it with you (e.g., a conflict that occurred or a nightmare or other distressing event), you will be charged half your session fee. You will be reminded of this policy before any charge to your account has been made.
- If you become involved in legal proceedings that require our participation, you will be expected to pay for my professional time even if we are called to testify by another party. Because of the difficulty of legal involvement, I charge $250 per hour for preparation and attendance at any legal proceeding.
Credit Card Authorization Policy:
Arya Therapy Services, LLC requires a credit card authorization be on file so that your balances can be settled as they occur. When credit card charges for unpaid balances arise (e.g., missed appointments), a statement of the charges and receipt may be mailed to you upon request. If you refuse to provide a card on file and you fail to provide 48 hours notice before canceling an appointment, you will be billed via invoice and charged an additional fee of $25.00 unless other arrangements are made in advance.
